Elena Fernandez, Chief Programs Officer at St. John’s Community Health (St. John’s), leads program development to advance the agency’s mission. She manages more than 11 integrated programs focused on social determinants of health, including Transgender, PRIME, Street Health, CalAIM, RISE, Domestic Violence, Behavioral Health, Mayor’s Fund, and Homeless services, all aimed at improving access to care.
With a visionary approach and unwavering commitment to community well-being, Ms. Fernandez has greatly expanded the reach and effectiveness of these crucial initiatives. Her leadership is marked by a collaborative spirit and an ability to foster innovation within complex systems of care. Under her direction, St. John’s has become a model for addressing the multifaceted needs of diverse populations, ensuring that underserved groups receive comprehensive, culturally competent services. Elena holds distinguished degrees from UC Berkeley and the University of Southern California. As a licensed clinical social worker, she also brings over fifteen years of dedicated teaching experience in mental health and community organizing, demonstrating both expertise and unwavering commitment to her field.
